We have an opportunity for two Behavioral Health Case Managers (CMs) in Charlotte and Raleigh North Carolina. 

Key duties and responsibilities:

  • Tracking medical status of assigned patients.
  • Responsible for medical charting in electronic CM database.
  • Request profile exams/evaluations and audiology exams at physical exam center.
  • Coordinate patient treatment plans with appropriate medical facilities.
  • Maintain automated referrals.
  • Submit case processing, medical documentation, and profile concerns to provider.
  • Attend health assessment events as determined by the needs of the state and the individual units.
  • Ensure patients referred by a provider are followed by the case management team.
  • Ensure all medical documentation received from the patient and/or their medical team is received and placed in appropriated electronic record.
  • Review and track health assessment lab results, contact patient and educate on
  • Track appointments by communication with unit to ensure patient attendance and submission of follow up documentation lifestyle modifications.

 Education and Experience

  • Bachelor’s or Master’s degree in Social Worker or Psychology from an accredited program.
  • Requires at least three years experience for Bachelor degree candidates or one year for Master’s degree candidates.

Work Hours

  • Minimum 40 hours per week
  • Scheduled shifts are normally days Monday through Friday.
  • Some weekend shifts required (1-2 times a quarter)
